INTRODUCTION: Acute appendicitis and symptomatic biliary stones are the most common indications for non-obstetrical surgical procedures during pregnancy. However, combination of these two clinical presentations in the same gestational period is anecdotal. CASE REPORT: A 30-year-old twin pregnant patient complicated with acute appendicitis followed by cholecystitis was reported with poor fetal outcomes. CONCLUSION: Despite appropriate management when two conditions, appendicitis and cholecystitis, occur in the same gestational period one after another complications might become inevitable.
